Case History: 66 year old woman presented with a history recent worsening of dyspnea and exert ional fatigue leading to hospital admission, she had mild long-standing exercise intolerance. She has a history of congenital heart disease and was told as a child she wouldn't survive surgery. She has had two prior strokes and is hypertensive and has thrombocytopenia.

The cardio-thoracic ratio is 55percentage. There is a wide aortic arch. The right long is moderately hyper inflated. She has the right aortic arch seen in 25percentage of tetralogy patients. There is a left long is smaller with an opacity in mid and upper long calcification is seen in region of aortic and pulmonary valve on the lateral field.
