Case History :

28 year old female house wife has a one year old son.A murmer was noticed in her when she was 18 years old. During the prenatal period her pregnancy complicated by difficult to control high blood pressure. fetal distress let to cesarean section.Presently the female has mild exertional dyspnea and is not on any medication.

On examination:
Appears well, not cyanotic. Pulse 75/min regular ; BP 180/100 mmHg right arm. JVP normal. Weak femoral and pedal pulses. Normal apical position. S1 normal, S2 widely split with normal movement. A2 loud. Grade 2-3 ESM maximal ILSB. No extra sounds or diastolic murmur. Chest clear.

Her leg BP is lower than the right arm. Clinical exam was diagnostic of coarctation. There is proximal hypertension, radiofemoral delay, and weak pulses in the femorals and beyond.
