Case Title: 20 year old woman known case of 'congenital heart disease' with COPD
Case History: 20 year old woman known case of 'congenital heart disease' with COPD, presented with dyspnea on exertion and fatigue. On examination she was mildly cyanotic with clubbing. Pulseoximeter shows saturations around 86% on low flow oxygen 0f 2l/min. On auscultation she had Grade 3 very long ejection systolic murmur heard best 2LICS.

The overriding aorta is clearly visible. The aortic valve cusps themselves are calcified with restricted opening. The ventricular septal defect is clearly seen below the level of aorta. There is both right and left ventricular hypertrophy and the right ventricular function is diminished. Click here to view movie
The pulmonary valve is markedly thickened with severe restriction of opening at the valve cusps. In addition the valve cusps are echo-bright suggesting calcification. Click here to view movie
