DAY 1 HIGHLIGHTS

Atorvastatin exerts a rapid, direct effect on GTP- cyclohydrolase I expression in human arteries leading to a striking improvement of vascular redox state

Atorvastatin protects the integrity of adherens junctions in endothelial cells by inhibiting VE-cadherin tyrosine phosphorylation

Soluble Lectin-like Oxidized LDL Receptor-1 and High Sensitivity Troponin T when measured in combination, improved the accuracy of ACS diagnosis in emergency rooms

REACH Registry: 22% excess in long-term adverse clinical events in patients with established atherosclerosis due to lack of use of evidence-based secondary prevention therapies at 1 year

Levels of class A macrophage scavenger receptor SR-A increase in unstable atherosclerotic plaque and peripheral blood mononuclear cells in ACS patients

Septolateral LV mechanical dyssynchrony is a strong and independent predictor of long term survival in systolic heart failure patients independent of QRS duration

Major life events in the year prior to acute myocardial infarction are associated with worse 1-year outcomes

In chronic heart failure patients, therapy guided by NT-proBNP leads to greater improvements in quality of life, than standard management therapy

Elevated oxidative stress in patients with normal left ventricular ejection fraction independently correlates with left ventricular mass index

Hemodynamic correction through ventricular assist device implantation modifies the altered bone turnover, in patients with advances heart failure

Elevated circulating levels of sCD93 are associated with clinical outcomes in patients with acute myocardial infaction

Addition of very low dose rivaroxaban to dual antiplatelet therapy in patients hospitalized with a recent ACS significantly reduced mortality, and ischemic events for a median of 1.1 years with a significant increase in non–CABG-related TIMI major bleeding and ICH.

Addition of vorapaxar to dual antiplatelet therapy in patients hospitalized with a recent ACS does not significantly reduce ischemic events, but increases bleeding events.

Use of bivalirudin alone results in similar ischemic events and less bleeding compared with unfractionated heparin/abciximab, in patients with NSTEMI

Administration of intracoronary abciximab compared with intravenous abciximab failed to improve composite clinical outcomes or reduce infarct size, in STEMI patients undergoing primary PCI

Extended duration of VTE prophylaxis with oral apixaban is not superior to in-hospital subcutaneous enoxaparin in patients admitted with acute medical illness and is associated with an increased risk of bleeding

Pounds Lost trial: A high carbohydrate diet may be more effective in weight loss and reducing insulin resistance in subjects with IRS1 rs29436410 CC genotype than those without this genotype
The highest carbohydrate diet group (both P for interaction <0.02) showed continued significant changes in insulin (p=0.007) and HOMA-IR (p=0.01).

Significantly lower levels of circulating EPCs and reduced functional properties in patients with diabetes when compared with the non-diabetics
HgA1C decreased from 9.5% to 8% and circulating EPCs increased to a level similar to the non-diabetic group after the tight control period.

The Anticoagulation and Risk Factors in Atrial Fibrillation (ATRIA) Study: 30-day mortality from ischemic stroke vs. intracranial hemorrhage in a large community-based cohort of patients with atrial fibrillation
1.6 times more lethal intracranial hemorrhage s occurred on warfarin than ischemic strokes occurring while off warfarin.

Coronary Optical Coherence Tomography Image Analysis: Significantly more frequent incidence of microchannels was reported in diabetics
None of the coronary risk factors (hypertension, dyslipidaemia and smoking) other than diabetes showed a significant relationship with microchannel formation.

Campania Salute Network study: Hypertensive patients with concomitant obesity have a highly increased prevalence of LVH and a modest increased prevalence of carotid plaques
Concomitant obesity was associated with a 21% increased prevalence of carotid plaques, a 3.7 times higher prevalence of LVH and a 2.4 times higher prevalence of combined vascular and cardiac target organ damage.

Goldn Study: Fenofibrate as an effective treatment for hypertriglyceridemia and in improvement of insulin resistance
Association between ITPR2 and waist-to-hip ratio was found and is highly expressed in human adipocytes.

AIDA STEMI Trial: The Abciximab Intracoronary versus intravenously Drug Application in ST-Elevation Myocardial Infarction
Intracoronary abciximab bolus application during PCI results in high local drug concentration, improved perfusion, reduction of infarct size and less microvascular obstruction in comparison to intravenous bolus application.

The ISAR-REACT 4 Trial: Bivalirudin is superior to abciximab + unfractionated heparin
Bivalirudin might be the preferred drug in patients undergoing PCI for an acute myocardial infarction with or without ST-segment elevation.

ADOPT (Apixaban Dosing to Optimize Protection from Thrombosis) Trial: extended use of apixaban is more effective than short term use of enoxaparin
Administration of oral apixaban (2.5 mg BID) reduces the incidence of VTE in acutely ill medical subjects for 1 month.

TRA•CER Trial: The Thrombin Receptor Antagonist for Clinical Event Reduction in Acute Coronary Syndrome
Compared to 16 percent for patients given the placebo, patients on vorapaxar also had a 14.7 percent risk of cardiovascular death, heart attack or stroke, a difference that was statistically insignificant.

The GenSalt Study: Salt sensitivity of blood pressure decreases progressively with higher levels of physical activity
Among the lowest to the highest quartiles of physical activity, participants having high salt sensitivity of systolic blood pressure showed decreased age- and sex-adjusted proportions of high salt sensitivity with increasing activity.

Antithrombotics and Antiplatelets Take the Center Stage Again at AHA 2011

Today's Spotlight

Major life events occurring prior to AMI, worsen the 1-year outcomes during recovery
Patients with ≥2 events before AMI experienced greater risk of rehospitalization, poorer cardiac-related quality of life, poorer general physical and mental functioning, and more depressive symptoms
